Select the category in each column that best matches your address fields. Select "Do Not Include" if the column is not an applicable field.

Please check if you wish to add "Current Resident". This will add "Current" for First Name and "Resident" for Last Name if no name is entered in name fields. It will add "or Current Resident" directly after the "Last Name" field if a name is entered in any name field.
